Calcium-channel blockers (CCB) were first developed in the 1960s. Since then, the list of formulations and variety of uses has increased significantly. Unfortunately, toxic exposures to CCBs have also increased since their debut. Emergency department management of calcium-channel blocker, beta blocker, and digoxin toxicity.

While it is relatively uncommon, an overdose of calcium-channel blockers, beta blockers, or digoxin has a significant morbidity and mortality rate, and its management can be complex. Digoxin toxicity can present with an acute overdose or as chronic toxicity while a patient is on therapeutic dosing, which has implications for diagnosis and management. While the patient's specific clinical presen...

Noncardiogenic pulmonary edema complicating massive diltiazem overdose.

Non-cardiogenic pulmonary edema has not been previously described in calcium channel blocker overdose. We describe a case of non-cardiogenic pulmonary edema occurring during the course of therapy for massive diltiazem overdose in a young patient with anorexia nervosa. Review of the current literature suggests that major and minor pulmonary complications occur with some frequency in the setting ...

Beta-Blocker Overdose Treated with Extended Duration High Dose Insulin Therapy

Poisoning and overdose of beta-blockers may be associated with significant morbidity and mortality. Management is often complicated by failure of initial first-line interventions in severe ingestions. High-dose insulin, or hyperinsulinemia/euglycemia therapy has been well described in calcium channel blocker overdose as well as mixed ingestions of calcium channeland betablocker agents. The data...

A multifaceted approach to calcium channel blocker overdose: a case report and literature review

Calcium channel blocker toxicity can be devastating. Initial therapy with fluid, calcium, and adrenoreceptor agonists should be prompt and novel therapies can be added if no response. Determining cardiogenic shock versus vasoplegia with echocardiogram or other hemodynamic monitoring may guide treatment options.
